Concierge Services are available for individuals who require services in their home or in the office.
Confidentiality is always a priority.
Billable items include and are not limited to the following:
- History and Physical
- Consultation
- Treatment plan
- Telephone, E-mail, text interaction with client or representative
- Blood draws and lab tests
- Medications
- Travel time – from our office to client’s home and back to the office.
PLEASE NOTE
- A deposit for initial consult is required
- There is a customary hourly fee with a minimum of 1-hour charge if client is seen at home
- Annual Membership Fee Required
